This form is used to formally request an extension, makeup work, or accommodation related to coursework, assessments, or approved extenuating circumstances.

All requests must be submitted through this form and will be reviewed by your assigned Clinical Supervisor in accordance with Millennium Training and UCASS academic policies. Supporting documentation is required where applicable.

Requests must be submitted within the required timeframe as outlined in the Makeup Work and Retake Policy. Submissions made outside of policy guidelines may be denied.
